No  passport applications will be accepted after 4 p.m.- except by appointment

Application for U.S. Passport or Registration -- DS-11
Application for U.S. Passport by Mail (Renewal Form) -- DS-82
U.S. Passport Amendment Application (Name Change) -- DS-5504
    (can only be used within 1 year of passport issue date) 
Statement Regarding Lost or Stolen Passport -- DS-64 ONLY if Passport has not expired

REQUIREMENTS FOR ORIGINAL PASSPORT:

The passport process takes approximately six to eight weeks.
Application for U.S. Passport or Registration -- DS-11
Birth Certificate with raised seal, can be original or certified copy; only. Must contain both parents names. 
Two (2) pictures that meet passport requirements - available at the County Clerk's Office for $10.
Driver License for proof of identification

IF APPLICANT UNDER 16 YEARS OF AGE—application requires both parents’ signature and both parents must present picture identification.  If only one parent can apply, applying parent/guardian must present affidavit from non-appearing parent.

REQUIREMENTS FOR RENEWAL OF PASSPORT:

Applicant must be able to submit previous passport
Applicant must have been at least 16 years old when most recent U.S. passport issued 
Previous U.S. passport was issued less than 15 years ago
Same name used OR name was changed by marriage or court order and proper documentation is submitted to reflect name change.
